LungLink Clinical Navigator II

Position Summary:
The LungLink Clinical Navigator II plays a key role in delivering connected respiratory care
by interpreting lung function and blood oxygen data transmitted through the LungLink app.
This position emphasizes both clinical precision and compassionate, patient-centered
engagement. As part of a multidisciplinary team, the Clinical Navigator II collaborates with
providers and care teams to help execute care protocols for patients with chronic
respiratory conditions.
Key Responsibilities:
• Monitor and interpret real-time spirometry and SpO₂ data via the LungLink platform.
• Identify trends, escalate abnormal findings, and document clinical insights.
• Communicate with patients to reinforce care plans, answer clinical questions ,and
ensure engagement.
• Coordinate with primary and specialty care practices to support protocol-based
decision making.
• Review patient trends monthly and contribute to care summary documentation.
• Document patient progress and interaction within the electronic medical record.
• Adhere to HIPAA, CMS, and internal privacy and documentation standards.
• Support the development and refinement of LungLink protocols and workflows.
• Mentor new staff and provide peer support as needed.
Minimum Qualifications:
To qualify, candidates must meet all of the following:
• Registered Respiratory Therapist (RRT) in good standing, licensed in at least one
U.S. state.
• Meet one of the following criteria:
•Hold a graduate-level degree (Master’s or higher) in a health-related or clinical
field;
•Have passed the Canopy or equivalent medical foreign language proficiency exam
•Have completed at least 4 years as a LungLink Clinical Navigator with a
demonstrated track record of clinical performance and patient connection.
Preferred Skills and Qualities:
• Strong interpersonal and communication skills
• Comfortable using digital tools and telehealth platforms
• Compassionate, organized, and detail-oriented
• Bilingual or multilingual communication skills are a plus
• Familiarity with value-based care, chronic disease management, RPM (Remote Patient
Monitoring), and remote therapeutic monitoring.
